Our district nursing team are based at the Community Hospital and
work closely with the surgery. They provide continuous care to people
of all ages in the community as well as the residential homes in
Llan Ffestiniog. As well as this they run a Wound Clinic which is
held in the Ganolfan each day from 11.30 to 12.30 by appointment.

The District Nursing Service provides a broad range of nursing expertise
within the home environment, private or local authority accommodation,
health centres/clinics and general practitioner surgeries, primarily
to all patients who are unable to attend/access general practitioner
surgeries/practice nurses.
To organise and deliver skilled evidence based nursing care that
is necessary to sustain patients and support carers in their own
homes or in residential accommodation.
Our aim is to promote and teach self-care whenever possible leading
to personal independence.
District Nurse Teams are attached to General Practitioner Practices
and are led by a District Nurse who is specially qualified to assess
your individual needs. The team includes qualified nurses and trained
Health Care Assistants. You may also receive care from nurses in
training under the guidance and supervision of your District Nurse.
Following referral the patient may expect:
- A personalised assessment of needs and development of an action
plan to meet those needs.
- Skilled nursing care for people who are acutely ill, terminally
or chronically ill, but who do not require hospital admission.
- Teaching and supporting patients and their carers, empowering
them to regain independence in everyday activities.
- Advise on personal health and safety in the home.
- Referral to other agencies with the individual's consent.
- Once an episode of care/treatment is completed the patient will
be discharged from the care of the District Nursing Service.
Referral to the District Nursing Team can be made through the Sector
Offices during office hours, Monday to Friday 9am - 5pm.

A 48-hour notice period is required to enable the initial District
Nursing visit to be planned.
Anyone can ask for advice from the District Nurse or for a District
Nurse to visit if they, or someone they care for, has a nursing
need. The District Nurse will still respond to requests to visit
within 48 hours. Should you wish to contact the District Nurse outside
office hours, that is during weekends or bank holidays please contact
Ambulance Control on 01248 689089 who will relay your message to
the relevant District Nurse.
If the District Nurse finds that you may need help from another
service, he/she will advise you accordingly, or refer you directly.You
can contact the District Nurse through your Doctor's surgery or
through one of the sector office.
REMEMBER - you do not need your Doctor's permission
to speak with the District Nurse.
